How Can I Update & Customize Fields on an Intake Form?

Accessing Your Intake Form Fields

  1. Go to Settings → Channels → Intake Forms.

  2. Click the Edit button next to the Intake Form you want to update.

  3. Select the Fields tab at the top of the form editor.


Managing Form Elements

Add a New Field

  1. Hover over the space above or below any existing element and click Add Element.

  2. In the pop-up window, configure your field or header details.

  3. Click Save.

Edit Existing Fields & Translating Field Labels

  1. Hover over the element you want to update and click the Pencil (Edit) icon on the right.

  2. Update the label, default text, or field settings in the edit window.

    • Translating Labels: To display form fields in another language, replace the default English text in the Label box with your translated text (e.g., change "First Name" to "Prénom").

  3. Click Save.

Note on Multilingual Forms: Intake forms do not support dynamic auto-translation or automatic language toggling. Translating a field label manually overrides the text for all viewers. If you need intake forms in multiple languages, create separate forms for each language (e.g., one English form and one French form) by manually translating the labels on each.

Delete a Field

  1. Hover over the element you want to remove and click the Trashcan (Delete) icon.

  2. Click OK in the confirmation window. (Caution: Deleting a field is permanent and cannot be undone.)
