You cannot add images/logos to the Intake Form itself. However you can embed them on your company Website which would have all the relevant branding/logo.

You can also personalize what the Customer sees after submitting the Intake Form, either adding a logo/image there or redirecting to your Company website. This can be done in Settings>Tools>Intake Form> Edit>Submit Settings.
